A 62-Year-Old Man With Obesity, Type 2 Diabetes, Fatty Liver And NASH Got A New Way Of Life Through Weight-loss Surgery

Pune, 18th April 2022:- A team headed by Dr Kedar Patil, Bariatric Surgeon, Apollo Spectra Hospital, Pune gave a fresh lease of life to a 62-year-old man weighing 90kgs and having Type 2 Diabetes, Obesity, Psychiatric illness, and Non-alcoholic steatohepatitis (NASH). The patient underwent Bariatric Surgery to lose weight, get his diseases controlled and lead a healthy life. After surgery he lose 7 to 8 kgs Weight. He has been successfully discharged from the hospital and has resumed his daily routine now.

 

 

 

Mr Ramesh Bhosale (Name Changed), a resident of Pune had a weight of 90 kgs and a BMI of 32.6 kg/m2. Being overweight and having multiple health issues, he experienced early fatigue, discomfort and couldn’t do his daily activities with ease. He had tried diet and exercise regimen for 2 years followed by weight loss medications for three months. Despite optimum and appropriate Medical management there was no significant weight loss or control of Diabetes .The patient was then referred to Dr Kedar Patil .

 

 

 

Dr Kedar Patil, Bariatric Surgeon, Apollo Spectra Hospital, Pune said, “This patient had Type 2 Diabetes Mellitus for 6 years. He also had morbid obesity (BMI was 32.6 kg/m2)  psychiatric illness, Non-alcoholic steatohepatitis (NASH) all linked to obesity. His diabetes was poorly controlled due to obesity, HBA1C was around 10 in spite of taking anti-diabetic medications and insulin, weight reduction medications didn’t help much. The patient had Central Obesity with most of the fat deposited on the abdomen but normal size extremities which is a typical phenotype of Asians. His Body composition analysis revealed high visceral fat – 32.5 (25% being a higher range for his height and weight). This visceral fat (inside the abdomen around the intestines) is the root cause of Metabolic syndrome. Furthermore, Metabolic syndrome includes a host of conditions such as Type 2 Diabetes, Hypertension, Lipid abnormalities, and Ischemic heart disease .Non-alcoholic steatohepatitis (NASH), Nonalcoholic fatty liver disease (NAFLD) is also an entity mainly seen in obesity, often ignored in the past but now emerging as a major cause of liver cirrhosis and liver cancer in later life”.

 

 

 

Dr Patil added, “A Sleeve Gastrectomy i.e.; reduction in size of stomach was done laparoscopically ,which produces hormonal changes and causes weight loss. He is advised modified diet for a month and then can resume regular food. This surgery is done under General Anesthesia and usually requires an hour to perform, patients are usually discharged 2-3 days after surgery and resume their work by 7-10 days .

 

 

 

“60-80% of operated patients can have improvement/ resolution of Type 2 Diabetes and Hypertension with Bariatric and Metabolic Surgery. Metabolic syndrome and obesity associated  diseases like Joint pain, Obstructive sleep apnea, Fatty liver and PCOD can be resolved in 50-80% of patients with this surgery.
