Pune City’s First Robotic Revisional Bariatric Surgery Successfully Performed at Ruby Hall Clinic
Pune, 3rd March 2026: In a landmark advancement for Bariatric -weight-loss surgery in Western India, Ruby Hall Clinic has successfully performed Pune city’s first Robotic Revisional Bariatric Surgery. The complex procedure was carried out in February 2026 by Dr. Kedar Patil, Robotic Bariatric and Advanced Laparoscopic Surgeon, along with a multidisciplinary team. The patient had previously undergone a Laparoscopic Sleeve Gastrectomy approximately 1.5 years ago at another centre and later developed a moderately large hiatal hernia with acid reflux and symptomatic gallstones. Using advanced robotic surgical technology, the team performed a robotic conversion from Sleeve Gastrectomy to Roux-en-Y Gastric Bypass, along with hiatal hernia repair and cholecystectomy. The surgery was successfully completed, and the patient has recovered well.
Revisional bariatric surgery remains relatively uncommon and significantly more complex than primary bariatric procedures. Globally, revisional surgeries account for nearly 10–15% of all bariatric operations, with numbers steadily rising due to the growing volume of primary weight-loss surgeries performed over the last decade. India is witnessing a parallel rise in obesity, with urban prevalence estimates ranging between 30–40% among adults, leading to an increase in bariatric interventions. These revisional procedures done to address the problem of weight regain after a primary procedure, which may happen in about 15 percentage of patients due to various reasons. Most of the revisional procedures are done laparoscopically .
Revisional procedures are also done to address the problems arising out of the primary procedure as seen in this particular case. Revisional procedures are technically demanding because the anatomy has already been surgically altered, resulting in scar tissue formation and distorted tissue planes that increase operative risk. In this case, the patient required simultaneous management of a large hiatal hernia and symptomatic gallstones in addition to metabolic revision. The robotic platform provided enhanced three-dimensional visualization, superior instrument articulation, tremor filtration, and precise suturing capabilities, which are particularly crucial in reconstructive gastrointestinal surgery. The conversion to Roux-en-Y Gastric Bypass was carefully planned to address reflux symptoms and improve long-term metabolic outcomes, while the hiatal hernia repair restored anatomical integrity. The successful cholecystectomy further resolved the gallstone-related symptoms, enabling a comprehensive single-stage solution.
The patient was discharged in stable condition following an uneventful recovery. She has resumed oral intake as per bariatric protocol and continues to do well on follow-up. Her reflux-related discomfort has subsided, and she is expected to experience sustained metabolic and symptomatic improvement in the coming months. The smooth post-operative course highlights the benefits of robotic-assisted surgery, including reduced pain, minimal blood loss, faster recovery, and improved surgical precision.
Robotic revisional bariatric surgery remain rare, particularly outside major metropolitan hubs, as it requires advanced infrastructure, high surgical expertise, and seamless multidisciplinary coordination.
